The client does not recognize your item so it will not display.

There's a way to do custom items but you must use some existing ones as template (Like npcs) but you cannot change name, description, icon just same item with another id which is pointless

you have to put it on client side in etcitem.grp and itemname-e.dat too
i did it it's work, only a small errror for the moment, when i check the item in the inventory, it
indicates me "NoItemName" on the custom.

as earlier was mentioned you shouldn't touch ANY of client files according to EULA ...
