Is the quest exclamation point above an npc's head an abnormal_vfx or special_vfx ? or something else altogether.
If it is a vfx which one is it ?

So not *any* packet the server sends to the client making it draw an exclamation over its head ?

no, its full client side feature based on your level/class/npcid and some fields in questname-e.dat
