PhP Script - DB Connection!

Im Making a Vitality-Vote Event ...So....
im need a php script (i no have idea about php)

1. The Form Has, TextBox And One Button
2. Write Name In Text Box And Press Button
3. Button Take Name And Look In DB If Name Exist (Table characters)
4. If Name Exist Then Put In Table (vitevent) Field (vote_points) +1
